Family Case Management

WES Health’s Chicago programs provide holistic support for families and youth through Family Case Management for pregnant individuals and infants, and Teen Reach, an after-school initiative promoting academic success, emotional growth, and risk prevention for adolescents.

Family Case Management (FCM)

Family Case Management is a statewide program in Illinois designed to provide comprehensive service coordination aimed at improving the health, social, educational, and developmental needs of pregnant individuals and infants (0 - 12 months) from low-income families. 


The program focuses on assessment, linkage to essential services, and ongoing care coordination to support families during pregnancy and the early stages of their child's life.


Key Program Features

  • Continuous assignment of a dedicated case manager to each client for personalized support 
  • Comprehensive needs assessments to identify and address individual family requirements 
  • Development of individualized care plans tailored to the specific needs of pregnant individuals    and their infants
  • Education and screening for perinatal mood disorders with appropriate referrals to mental   health services when necessary 
  • Regular face-to-face contacts and home visits as outlined in the Program Policy Manual to ensure ongoing support and engagement 
  • Objective developmental screenings within the infant's first year using Medicaid-approved tools to monitor growth and development
  • Referral services to community providers, including primary care physicians and Medicaid  managed care entities, to enhance service integration and care coordination

Teen Reach Program

Teen Reach is a structured youth development program designed to address the needs of adolescents during critical after-school hours. The program focuses on providing resources and support that promote academic success, social-emotional development, and the prevention of high-risk behaviors.

Key Program Features

  • Structured activities during late afternoon and early evening hours on school days to mitigate risk-taking behaviors 
  • Provision of essential resources such as high-speed internet access, tablets or computers, and academic supervision to enhance learning 
  • Focus on academic achievement by offering tutoring, homework assistance, and enrichment opportunities 
  • Development of social-emotional skills through workshops, group activities, and mentoring relationships
